All-on-4 replaces a full arch of missing or failing teeth with just four implants — two placed straight at the front and two angled towards the back — supporting a fixed, non-removable set of teeth.
At PlanDental in Mahim West, All-on-4 treatment is planned and placed by Dr. Hrishikesh Naik and Dr. Nisha Naik, both MDS in Prosthodontics and Implantology. Read more about the clinic before you book.
Patients usually book this for: most/all teeth missing in a jaw limited bone wanting to avoid grafting tired of removable dentures comparing All-on-4 vs All-on-6
All-on-4 is a full-arch implant protocol that uses just four implants to support an entire fixed set of replacement teeth. The two back implants are placed at an angle to make use of the denser bone that's usually still available in that part of the jaw, which is why All-on-4 can often avoid the need for bone grafting even in patients with reduced bone volume. It's one of two full-arch options offered at PlanDental, alongside All-on-6.
The All-on-4 technique was developed to give patients with significant tooth loss a fixed, full-arch solution without necessarily needing bone grafting first. By angling the two posterior implants, the protocol makes use of available bone in the front of the jaw, where bone tends to be denser and more available even after long-term tooth loss. A fixed prosthetic arch is then attached to all four implants.
Four implants, angled posteriors, often avoids grafting.
Six implants for extra support and redundancy.
A non-surgical, removable alternative.
Detailed imaging confirms bone volume and plans the exact angle and position of all four implants.
Two implants are placed straight at the front, two angled towards the back of the jaw.
The implants integrate with the bone over several months before final loading.
A full fixed replacement arch is attached to the four healed implants.
All-on-4 cost in Mumbai depends on the materials used for the final prosthetic arch and whether both jaws are being treated. PlanDental doesn't publish a fixed figure here — call or WhatsApp and the clinic will talk you through it after the assessment.
This section describes what affects cost, not a quotation. Actual cost is confirmed only after the assessment.
Send your details and any recent X-rays on WhatsApp for a straightforward opinion on whether All-on-4 or a different option suits your case.
Soft-food diet and mild swelling are expected right after surgery.
Regular check-ups monitor integration of all four implants.
The fixed arch functions much like natural teeth for eating and speaking.
Periodic professional cleaning around the implants and prosthesis is important.
Very severe bone loss even at the front of the jaw may still require grafting or point toward a different plan.
With only four implants, losing one can affect the whole arch — this is why follow-up care matters.
Uncontrolled diabetes or heavy smoking can affect how well the implants integrate.
Implant positions are planned from your own scans, not a fixed template.
Neither is pushed by default — the right protocol is recommended after assessment.

The angled placement of the two back implants distributes bite forces efficiently and makes use of denser bone, which is why four implants can support a full arch reliably.
All-on-4 uses fewer implants and often avoids bone grafting, while All-on-6 uses six implants for extra support and redundancy. See Full Mouth Dental Implants for a full comparison.
Often not, since the angled back implants are designed to work with the bone that's usually still present at the front of the jaw — but this is confirmed with your own imaging.
Fixed. The prosthetic arch is attached to the implants and isn't designed to be removed by the patient day to day.
With good oral hygiene and regular professional check-ups, long-term studies report strong survival rates for the All-on-4 protocol over many years.
Yes — many patients have All-on-4 on one jaw, others on both, depending on which arch needs it.
